What to Expect
When You Visit
We accept a range of major insurance plans and are happy to verify your coverage before your visit. Please contact our office to confirm participation with your specific plan. Self-pay and cash-pay options are also available.
- Photo identification
- Insurance card and co-pay
- List of current medications and dosages
- Primary care referral (if required by your plan)
- Completed new patient forms (sent ahead of your visit)
Our standard office hours are Monday through Friday, 9:00 AM – 5:00 PM. Weekend and after-hours appointments may be available by arrangement. Please call us to discuss scheduling options that work for you.
We kindly ask for 24 hours' notice if you need to reschedule or cancel an appointment. This allows us to offer the slot to another patient who may need care. Thank you for your understanding.
